### General Terms and Conditions

1. Services Provided:

- Student Life Coach Agency ("SLCA") offers educational and mentorship services to middle school and high school students.

- Services include monthly grade updates, personal mentorship, language support, college tours, and academic achievement prizes as described in the brochure.

2. Payment and Subscriptions:

- Clients must select a subscription plan (Subscription 1 or Subscription 2) and agree to pay the monthly fee as specified.

- Payments are due on the first day of each month and can be made via the SLCA website or by contacting our office directly.

- SLCA reserves the right to adjust subscription fees with a 30-day notice to clients.

3. Cancellation and Refunds:

- Clients may cancel their subscription at any time by providing written notice to SLCA.

- Refunds are not provided for partial months. Refunds for unused portions of prepaid subscriptions may be issued upon cancellation at the discretion of SLCA management.

4. Conduct and Expectations:

- Students are expected to actively participate in mentoring sessions and educational activities.

- Respectful behavior towards mentors, staff, and peers is mandatory. SLCA reserves the right to terminate services for any student exhibiting disruptive behavior.

5. Privacy Policy:

- SLCA respects client privacy and confidentiality. Information collected is used solely for providing educational services and will not be shared without consent, except as required by law.

6. Liability Waiver:

- Participation in SLCA activities, including college tours and mentorship sessions, carries inherent risks. Clients agree to release SLCA and its employees from liability for injuries or accidents during these activities, except those caused by gross negligence or willful misconduct.

7. Amendments:

- SLCA reserves the right to amend these terms and conditions. Clients will be notified of changes via email or through the SLCA website.

8. Governing Law:

- These terms are governed by the laws of the State of North Dakota. Any disputes will be resolved in the courts of North Dakota.

By signing below, I acknowledge that I have read and agree to abide by the terms and conditions set forth by Student Life Coach Agency.

Child Waiver

I, the undersigned parent/guardian of the above-named child, hereby acknowledge and agree:

1. Assumption of Risk: I understand that participating in SLCA activities involves risks, including educational sessions, mentorship activities, and college tours.

3. Release of Liability: I release SLCA, its employees, agents, and representatives from liability for injuries, damages, or losses that may occur as a result of my child's participation in SLCA activities, except those caused by gross negligence or willful misconduct, we also do not guarantee your child goes to college or improvements which can only be done through applying themselves, we will only advice , we also have no professional licenses and do not require our employees of such either.

3. Medical Authorization: In case of emergency, I authorize SLCA staff to seek medical treatment for my child, including transportation to a medical facility if necessary. SLCA will make reasonable efforts to contact me in such situations.

4. Photography and Publicity: I grant SLCA permission to use photographs or video recordings of my child for promotional purposes, including website content and social media.

5. Agreement to Terms: I have read and understand the terms of this waiver and agree to its provisions on behalf of my child.
